A small number of 2018 BMW 540d xDrive vehicles, equipped with Exhaust Gas Recirculation (EGR) modules could experience an internal glycol coolant leak that may result in an engine fire.

BMW first became aware of the problem in October 2018 when they opened an investigation into reports of certain BMW vehicles experiencing engine fires. What they found was; that these vehicles were equipped with an Exhaust Gas Recirculation (EGR) module with an integrated cooler that could leak coolant internally. The coolant mixed with diesel engine soot, combined with the high EGR temperatures could result in smoldering, fire, and a melted intake manifold.

During vehicle operation, the driver may be alerted there is a problem through:

  • A warning symbol in the instrument cluster displaying a loss of engine coolant.
  • A reduction in engine power.
  • An unpleasant exhaust odor.
  • Unusual noise from the engine compartment.
  • Smoke from the engine compartment.

NHTSA campaign number 18V-755 – BMW recalled 44,368 vehicles.

This recall campaign affected certain 2013-2018 BMW vehicles with the N47T and N57T diesel engines that were produced from September 12, 2012, to June 29, 2017.

NHTSA campaign number 21V-907 – BMW recalled 50,404 vehicles.

After continued field monitoring and review of field data, BMW determined that the EGR cooler may not be sufficiently durable to last the lifetime of the vehicle. They decided to issue a recall that would replace the EGR module on all potentially affected vehicles, with an improved cooler design. The intake manifold was inspected and replaced as necessary, and the EGR pipe connecting the cooler to the manifold was cleaned.

NHTSA campaign number 22V-614 – BMW is recalling 298 vehicles.

The vehicles subject to this recall have a different design and manufacturer of the EGR cooler than the vehicles previously recalled. After further monitoring and review of field data, it was found that the specific EGR cooler for certain 2018 BMW 540d xDrive vehicles may not be robust to last the vehicle’s lifetime. BMW has decided to perform a recall for these vehicles as well.

Owners receiving notices will be asked to return to their BMW dealer to have the EGR module replaced and the intake manifold inspected and replaced as necessary.

 Automotive Defect Investigation

In 2017, the NHTSA Office of Defects Investigation (ODI) opened two Recall Queries (RQ) affecting certain Hyundai and Kia vehicles equipped with “Theta II” engines. The investigation looked at both the timeliness and scope of the recalls as well as the compliance of each company to meet their reporting requirements under the Motor Vehicle Safety Act. Numerous violations of the requirements were found for both companies, and in November 2020, both companies entered into Consent Orders with NHTSA to resolve NHTSA’s assertions of violations of law.

In 2018, the Center for Automotive Safety (CAS) petitioned the Agency to investigate non-crash fires on various Hyundai and Kia models, alleging vehicle fires at varying points of origin and occurring under various non-crash related circumstances (including key-off fires). The ODI found that many of the fire incidents appeared to start in the engine compartment area and made reference to the two open RQ investigations as likely pertinent. After reviewing responses two Preliminary Evaluations were started to investigate incidents of non-crash fires (regardless of origin or operational status of the vehicle) on various model-year Hyundai (Sonata and Santa Fe) and Kia (Optima, Sorento, and Soul) models.

To date, Hyundai and Kia have issued several recalls addressing vehicle fires, including those due to engine failure involving various engine types (Theta II GDI, Theta II MPI, Theta II MPI HEV, Nu GDI, and Gamma GDI). The repair for the engine failure recalls typically involves engine inspection (to detect existing damage), and if necessary, engine replacement. Additionally, both manufacturers are installing an engine control software modification known as Knock Sensor Detection Software (KSDS) which is intended to detect impending engine failure, alert the driver, and limit engine power (to lessen the likelihood of engine failure). KSDS is being installed under recalls and non-safety field actions, and both companies have extended limited engine warranties. They have even replaced engines if the KSDS software detects an impending engine failure.

The Office of Defects Investigation (ODI) is opening this Engineering Analysis for two primary purposes. First, the investigation will continue to evaluate the scope of recalls initially begun under prior ODI actions, and will also include other recalls for fires due to engine failure. Second, the investigation will monitor and evaluate the efficacy of the recall remedies for engine fire-related recalls, as well as the long-term viability of related programs and non-safety field actions being conducted by Hyundai and Kia.

General Motors have identified a problem affecting certain 2017-2019 Chevrolet Silverado and GMC Sierra 2500/3500 trucks manufactured with the Duramax diesel 6.6 litre engine and the optional block-heater power cable.

Contamination of the sealing surface of the block-heater heater element can allow a coolant leak to develop. If leaking coolant contacts the block heater cable terminals while the cable is plugged into an energy source a short could occur.

In February 2021, a GM quality manager submitted a report to GM’s Speak Up For Safety program after inspecting an engine-block heater recovered from a 2019 model year Chevrolet Silverado 3500. The owner reported sparking from under the hood after the engine block heater was plugged in. The block heater was leaking coolant, and the engineer observed thermal damage at the connection between the heater and the block heater power cable. This vehicle had previously been fixed for this condition under NHTSA recall 19V-328.

Further investigation identified nineteen potential fires involving engine-block heaters in vehicles remedied under 19V-328. Based on this data, GM opened a formal product investigation and found 122 complaints and 24 reports of potential fires between December 3, 2019 and May 28, 2021 involving vehicles that received the recall remedy under 19V-328. Based on this data GM decided to conduct a safety recall on these vehicles.

Dealers will disable the block heater. GM will provide free replacement block heaters and cords under a separate customer satisfaction campaign. For vehicles previously included in recall 19V-328, the heater cord will be replaced and rerouted. Owner notification letters are expected to be mailed August 16, 2021. GM’s number for this recall is 212329840 and the NHTSA campaign number is 21V-496.
