A critical safety concern has been identified in some 2025 Chevrolet Equinox EV all-wheel drive vehicles. The problem affects the adaptive cruise control (ACC) system which may fail to engage the brakes as expected. GM first became aware of the issue in October 2024 when a GM engineer reported an incident involving a test vehicle through the company’s Speak Up For Safety (SUFS) program. The report indicated that the ACC system on a 2025 Equinox EV failed to bring the vehicle to a complete stop as anticipated. Following this, GM initiated an investigation, which revealed a software calibration issue in the brake system control module (BSCM) specific to all-wheel drive models not equipped with Super Cruise.

The investigation found that the incorrect software calibration prevented the BSCM from properly receiving commands from the ACC feature to apply brakes. This critical malfunction can lead to situations where the vehicle does not sufficiently slow down or stop when adaptive cruise control is engaged, relying on the driver’s timely intervention to avoid potential collisions.

While GM has confirmed five field complaints related to this condition, fortunately, there have been no reported accidents or injuries associated with it. However, the potential for increased risk due to delayed or insufficient braking when relying on the ACC feature is a serious safety concern that necessitates immediate attention.

Drivers of affected vehicles may notice instances where the vehicle does not slow down adequately when adaptive cruise control is engaged. In such cases, it is crucial for drivers to manually apply the brakes to safely slow or stop the vehicle, mitigating the risk of a crash.

The Office of Defects Investigation of the National Highway Traffic Safety Administration (NHTSA) have opened an investigation (PE21-020) into crashes involving first responder scenes and vehicles manufactured by Tesla. The vehicles involved were operating in either Autopilot or Traffic Aware Cruise Control prior to the accident.

2017-2021 Tesla Model 3
2014-2021 Tesla Model S
2015-2021 Tesla Model X
2019-2021 Tesla Model Y

Since January 2018, the NHTSA has identified twelve crashes in which Tesla models of various configurations, encountered first responder scenes and preceded to crash into one or more vehicles involved with those scenes. Most incidents took place after dark and included traffic control measures such as first responder vehicle lights, flares, an illuminated arrow board and road cones. The involved vehicles were all engaged in either Autopilot or Traffic Aware Cruise Control during the approach to the crashes.

Autopilot is an Advanced Driver Assistance System (ADAS) in which the vehicle maintains its speed and lane centring. With the ADAS active, the driver still holds primary responsibility for Object and Event Detection and Response (OEDR), e.g., identification of obstacles in the roadway or adverse maneuvers by other vehicles during the Dynamic Driving Task (DDT).

The NHTSA have opened a Preliminary Evaluation of the SAE Level 2 ADAS system (Autopilot) in the Tesla 3, S, X, and Y models. The investigation will evaluate the methods used to monitor, assist, and enforce the driver’s engagement with the dynamic driving task during Autopilot operation. The investigation will also assess the OEDR by vehicles when engaged in Autopilot mode and ODD in which the Autopilot mode is functional. The investigation will also include examination of the contributing circumstances for the confirmed crashes listed below and other similar crashes.

Chrysler will be contacting the owners of certain 2014-2016 Chrysler 300, 2014-2016, 2018 Dodge Challenger and 2014-2016 Dodge Charger cars regarding a problem affecting the cruise control system.

According to the defect report, the Mopar Stage 1 Kit Performance Packages Powertrain Control Modules (PCM) and Demon Crate PCM in these vehicles could have a defect that may prevent the cruise control system from disengaging. If there is a short circuit within the vehicle’s wiring while the cruise control is being used, the driver may not be able to shut off the cruise control either by depressing the brake pedal or manually turning the system off . During a short, the driver may be alerted through multiple warning lights illuminated on the instrument panel. If the vehicle continues to maintain speed or start to accelerate, the driver should shift to neutral and/or use the brakes to slow the vehicle and bring it to a stop.

Hyundai is recalling over one million vehicles for a defective stop lamp switch that could experience an intermittent switch point contact. This condition could result in intermittent operation of the push button start feature, the inability to remove the transmission from the “park” position, illumination of the Electronic Stability Control (ESC) warning lamp, intermittent interference with operation of the cruise control, and intermittent operation of the stop lamps. Safety advocates encouraging a recall of certain 2002-2004 Ford Escape vehicles are also asking the National Highway Traffic Safety Administration (NHTSA) to include the 2002-2004 Mazda Tribute in their investigation. According to the group, both sport utility vehicles (SUV) are similar vehicles built on the same assembly line. In 2004 Ford and Mazda issued identical recalls for unintended acceleration caused by an accelerator cable that may not return to the idle position. Ford later issued a technical service bulletin that warned dealers that an improper repair could lead to an adjacent cruise control cable being damaged and snagging on the engine cover causing unintended acceleration. The Center for Auto Safety say that because the vehicles are so similar and there is no way of knowing whether the recalls were carried out correctly, Ford and Mazda should issue a recall to ensure the safety of the vehicles.

The National Highway Traffic Safety Administration (NHTSA) has opened an investigation into certain 2005-2006 Ford Taurus vehicles after receiving 14 reports of stuck throttles. Consumer complaints alleged vehicle engines could rev as high at 4,000 RMP after shifting into “park” or “neutral”, and difficulty stopping the vehicle has resulted in dangerous situations for drivers. According to these reports, the problem could be the result of a stuck throttle due to the cruise control cable detaching. If this preliminary investigation should lead to a vehicle recall, an estimated 360,000 vehicles could be affected.
